Please choose the type of relationship you are requesting.  Charitable Organizations are groups that receive donations from Vendor participants in the program.  Donating Vendors are groups who wish to officially register their donations.

In order for us to complete your application and approve it, you must mail in the following documents:

If you currently receive CHSP funding, please submit:

Current CHSP Award Letter

If you do not currently receive CHSP funding, please submit:

  • A copy of your agency's most recent 990.
  • A copy of your agency's most recent audit.
  • Your agency's 501 c3 determination letter
  • A statement indicating the extent of your local direct service delivery.
